On the intake form you will be asked to list your D.O.B, city of birth, and exact time. I will need all of this information to give you an accurate reading. If you only know an estimate of your birth time I can work with that but the exact time would be ideal! :)
You can also list any specific questions you have, any topics you want to focus more time on, and anything you don't want to talk about.
Please note this is not a psychic or tarot reading. I will share intuitive insights with you about your life from studying your birth chart. This is incredibly valuable information to have because the more we can consciously align with our charts, the more fulfilled and successful we tend to be.
